Custom Fit Knee Replacement Surgery using CT Scan – The Latest Technology for Arthritis patients in North India

Personalized Knee Replacement using CT scan is set to change the way knee replacement is carried out in India. For the first time in North India, doctors at Fortis Mohali have successfully performed CT based custom fit total knee replacement surgery on a 53 year old patient.

The patient, 53 year old Mohini Taneja, was suffering from advanced arthritis for the last 15 years and had tried every possible non invasive method without any success until she opted to undertake this procedure. Within 30 hours of surgery, Taneja was back on her feet walking few steps and outside the room within 3-4 days with this new technology.

Doctors at the hospital say this kind of surgery increases the longevity of the joint replacement by more than 10 years, making it fit even for those in younger age groups.

Talking about the advantages for patients, Dr Manuj Wadhwa, Head Dept of Orthopaedics & Joint Replacement Fortis Hospital Mohali said, "This technique help implants last longer, prevent uneven wear and avoid revised surgery. It also results in increased mobility, a more ‘natural’ feeling knee, lesser pain and greater ease in performing normal daily activities. There are fewer complications and longer-lasting results."

The need for such a personalized surgery arises from the fact that no two knees are alike. There are variations in size, shape and the extent and kind of damage involved.

The customized procedure uses CT scan for taking the pictures of the joint, and an artificial joint is crafted based on the pictures. The surgery takes 15 to 20 minutes compared to more than an hour needed in the lengthy procedures that are required otherwise in fitting the joint. This new technique eliminates 22 steps during the surgery as compared to a conventional surgery. It also means reduced infections, anaesthesia requirements, blood loss, faster recovery and a longer lasting implant solution.

Explaining how the technique works, Dr Wadhwa said, "A 3D model of the knee is made through pictures taken by CT scan after taking into account the bone structure, weight ratios, gender, age and even mechanical alignment in order to determine the correct implant size and placement based on the patient's own knee anatomy. The images are then sent to bio-engineers in the United States where personalized moulds are created and shipped to India. This way precise bone cuts can be made by surgeons resulting in much better bone to implant fit. This technology ensures far greater accuracy and reduces the operation time by almost 40% and also results in less loss of bone & tissues."

Traditionally, the life of knee replacements are 10- 15 years, but with the advent of these new devices the life of knee replacements can be increased up to 25- 30 years.

The doctor added that so far, MRI was used for imaging, however, while MRI is good for imaging tissues, CT scan is best for imaging bones and gives the most accurate picture of the joint. The procedure is new and was launched only six months back in the USA.
